The episode opens with Armaan engrossed in a business meeting when Ruhi walks in, asking for his schedule diary on Abhira’s behalf. Armaan, preoccupied, tells her to take it herself. In doing so, Ruhi unknowingly picks up Akshara’s diary instead—neither she nor Abhira realizes the switch.
Later, as Ruhi flips through the diary, she is struck by a devastating truth: Arohi’s death was a consequence of her own actions. The emotional shock overwhelms her, triggering sudden labor pains. Abhira, noticing Ruhi in distress, immediately grows concerned. Meanwhile, Armaan suspects he may have left Akshara’s diary at the Goenka house and begins searching frantically.
Abhira quickly informs Armaan about Ruhi’s condition. Stunned by the news of early labor, he asks why it’s happening so soon. Abhira explains that Ruhi is in deep shock, likely causing the premature labor. She urges him to come to the hospital without delay. Though Ruhi initially resists getting into the car, Abhira manages to calm and convince her, determined to get her the help she needs.
As they rush to the hospital, Armaan is consumed by guilt, blaming himself for everything. At the hospital, Abhira stays on edge, trying to piece together what triggered Ruhi’s breakdown. Armaan, weighed down by remorse, attempts to come clean about the diary mix-up and its consequences.
Meanwhile, at the Goenka and Poddar households, Swarna, Manish, and Surekha notice that Ruhi and Abhira are missing. Vidya calls Manish to check on them and learns the same, sparking widespread concern among both families.
Ruhi’s condition worsens rapidly. A nurse informs Armaan and Abhira that she is struggling. Abhira, distraught, prays fervently for the safety of both Ruhi and the unborn child. Armaan finally confesses that Ruhi learned the truth about Akshara because of his carelessness. Abhira steels herself to handle what lies ahead.
As Ruhi is taken in for delivery, Armaan, overwhelmed and panicked, steps out of the hospital to collect himself—only to meet with an accident moments later. Vidya receives the shocking news. Inside, a nurse hands Abhira a consent form, forcing her to make an impossible choice: if complications arise, who should be saved—Ruhi or the baby? The question leaves Abhira shattered.
While the families rush to the hospital, Swarna and Vidya desperately search for Armaan. Manish silently prays for strength, while Kaveri is seen seeking divine intervention for her family’s happiness. Manish wonders whether Kaveri still holds onto her old views against surrogacy, even in this crisis.
